Abstract

Purpose) The effects of lamotrigine, an antiepileptic drug, was investigated for the treatment of attention-deficit hyperactivity disorder(ADHD) symptoms in pediatric parients with epilepsy. Methods) Ninety pediatric patients diagnosed epilepsy (Male : Female = 61 : 29, Mean age = 9.8 ± 3.8 yr) were retrospectively identified in Department of Pediatrics, Chonbuk National University Children’s Hospital, from September, 2010 to January, 2014. We performed a Korean version of ADHD rating scale(K-ARS)-Ⅳ that assesses two subscales (inattention and hyperactivity-impulsivity) was used to evaluate the ADHD symptoms before and after treatment of lamotrigine. Initial dosage of lamotrigine was 1mg/kg/day, titrated up to 7 mg/kg/day (or maximum 200mg/day). The mean interval of ADHD testing was about 12.3 months. Results) The epilepsy children with ADHD of our study, male patients was significantly higher occurrence than female by almost two fold (Male 61 : Female 29). We found that 50 of 90 patients (55.6 %) improved after lamotrigine, however, 35 of 90 patients (38.9 %) did not improved, 5 of 90 patients (5.6 %) did not changed with medication. Among the seizure-freedom group, lamotrigine showed significantly effective for ADHD symptoms (15.9±1.4 vs. 12.1±1.3, P 0.001). In ADHD diagnosed group, lamotrigine revealed significant improvement for ADHD scores (28.0±1.6 vs. 18.1±2.6, p 0.001). Conclusions) This study was the first report that lamotrigine was not only safe to use for ADHD in pediatric epilepsy patients, but also effective treatment of ADHD symptoms. And this study suggested the control of the seizure might be responding better to improvement of ADHD symptoms.
